    Seems there are a few concerns about the heat shield, which is
    a different design from the one used Apollo, and for which there
    are concerns about gases developing inside the shield, which can
    then blow off parts of the shield.

    Looks like normalization of deviance to me - shades of Challenger
    and Columbia. By the way, the "Columbia Accident Ivestigation Board"
    report is excellent reading. I printed it out when it came out,
    but it is still available online.
